Insurance plans are usually classified as either HMO (Health Maintenance Organization) or PPO (Preferred Provider Organization). What does Medical Necessity mean for a Breast Reduction According to CIGNA?

A breast reduction is deemed medically necessary when a patient’s macromastia (abnormally enlarged) breasts causes them to suffer physical distress that is not alleviated through non-surgical interventions. According to CIGNA breast reduction is only covered in women who are at least 18 years of age or who have completed breast growth and who suffer with documented symptoms (see below).

How to Use CIGNA Insurance to Cover a Breast Reduction?

During your consultation at HIM & HER Plastic Surgery, Dr. Vitali Azouz will perform a history and physical and document any symptoms you may be experiencing from your symptomatic macromastia. Your plastic surgeon will also document any remedies that you may have tried in the past to treat your symptoms. After performing a physical exam and taking pictures your surgeon will determine how much breast volume that would be removed in order to improve your symptoms.

What are the Documented Symptoms of Symptomatic Macromastia according to CIGNA?

According to CIGNA coverage policy on breast reduction surgery. A patient must have all of the following documented symptoms that are unresponsive to medical management:

  1. Pain in the upper back, neck and shoulders or ulnar nerve palsy for which no other etiology has been found on appropriate evaluation
  2. Intertrigo, dermatitis, eczema or hidradenitis at the inframammary fold

What are the Documented Physical Findings of Patients with Symptomatic Macromastia according to CIGNA?

According to CIGNA coverage policy on breast reduction surgery, preoperative photographs must be submitted that show the presence of:

  1. Significant breast hypertrophy
  2. Shoulder grooving from bra straps and/or intertrigo

How Much Tissue Needs to be Removed to Meet CIGNA Criteria for a Breast Reduction?

  • According to CIGNA coverage policy, the average grams of tissue to be removed from each breast must be greater than 1 kilogram or above the 22nd percentile of Schnur Sliding Scale.  The Schnur Sliding Scale is an evaluation tool that uses a patient’s body surface area (body weight and height) to determine the weight of breast tissue to be excised.
