Gynecomastia or male breast tissue is often caused by hormonal imbalances of estrogen and testosterone that occur during puberty. Estrogen is a hormone that is responsible for the development and growth of breast tissue, whereas androgens such as testosterone are hormones that inhibit breast tissue proliferation. Several classes of medications are associated with development of gynecomastia due to their effects on hormone levels. Identifying the cause of gynecomastia may be helpful in treatment options, which may include cessation of the medication and surgery. Here are some examples of medications that are associated with gynecomastia.
Anti-androgens increase the level of estrogen relative to androgen by blocking the effect of androgens or reducing their production. This imbalance leads to the development of gynecomastia. Anti-androgens are commonly used to treat a variety of medical conditions.
Finasteride and Dutasteride block the conversion of testosterone to dihydrotestosterone and are used to treat male pattern baldness and benign prostate hyperplasia (BPH). As the level of testosterone increases, the conversion of testosterone to estradiol increases in the circulation.
Spironolactone is used to treat high blood pressure and blocks the effects of androgens by binding to androgen receptors. Spironolactone also increases the conversion of testosterone to estrogen and estradiol. This results in a lower level of circulating testosterone and increases relative levels of estrogen and estradiol.
Flutamide and Bicalutamide are used to treat the proliferation of prostate cancer by blocking the action of testosterone and thereby increasing the level of estradiol.
Hormonal therapies directly or indirectly increase the level of estrogen, which can stimulate breast tissue development.
What are some commonly prescribed hormonal therapies and what medical conditions are they used for?
Anabolic androgenic steroids and testosterone therapy are used to increase performance and muscle mass; theywork by increasing the levels of circulating testosterone in the body. This can create a hormonal imbalance as excess testosterone is converted to estrogen which can result in the development and growth of breast tissue.
Some medications that reduce stomach acid production can lead to a hormonal imbalance by also blocking androgen receptors.
Cimetidine is used to treat ulcers in the stomach and duodenum and inhibits testosterone receptors on the breast leading to unopposed estrogen activity on the breast.
Omeprazole is a commonly prescribed medication that reduces acid secretion and is used to treat heartburn or gastroesophageal reflux (GERD). Omeprazole may increase levels of estrogen by interfering with liver enzymes that play a role in the breakdown of estrogen.
Antihypertensive medications are used to manage high blood pressure by blocking the pathways that cause blood vessels to narrow. These medications can also interfere with the production of testosterone causing a hormonal imbalance.
Calcium channel blockers, (e.g., Verapamil, Nifedipine) are commonly used to treat blood pressure by preventing the influx of calcium into heart tissues and thus lowering contraction and blood pressure. Calcium channel blockers may interfere with testosterone production and estrogen metabolism leading to a hormonal imbalance.
